or even complete A-V block. Two cases are reported in which catheter-induced transient high grade Mobitz II A-V block, acute reversible prolongation of infranodal conduction time with probable intra-Hisian conduction delay, and persistent (6 hours duration) complete A-V block occurred in patients with left bundle branch block. The observations made indicate that A-V conduction times and the appearance of "split His" potentials may be influenced by the recording catheter itself, and suggest that the operators be prepared to institute immediate right ventricular pacing when His bundle recordings are obtained in patients with left bundle branch block.

Catheter-induced 3:1 second degree atrioventricular nodal block during atrioventricular nodal reentrant tachycardia.

Second-degree atrioventricular (AV) nodal block with 2:1 AV conduction during atrioventricular nodal reentrant tachycardia (AVNRT) has been previously described. We report a patient who developed AVNRT during which 3:1 intra-infra-Hisian block due to catheter manipulation was observed. This was followed by gradual, incremental recovery of AV conduction. AVNRT accounts for approximately 50% of c...

Acute His Bundle Injury during Permanent His Bundle Pacing: Mechanistic Insights into Intra- Hisian Block

Permanent His bundle pacing (HBP) is a physiological alternative to right ventricular pacing. Acute injury to the His bundle can occur during permanent HBP in the form of transient or permanent bundle branch blocks. Observations of mechanical injury to the His bundle during HBP lead fixation and electrophysiological response to HBP provides additional mechanistic insights into the pathology of ...

Further observations on the electrophysiologic effects of oral amiodarone therapy.

A case is presented of a reversible intra-Hisian block occurring under amiodarone treatment for atrial tachycardia in a patient without clear intraventricular conduction abnormalities. His bundle recordings showed an atrial tachycardia with intermittent exit block and greatly prolonged BH and HV intervals (40 and 100 msec, respectively). Thirty days after amiodarone discontinuation, His bundle ...

Catheter ablation of ventricular ectopy with para‐hisian origin: importance of mapping both sides of the interventricular septum and understanding when to stop ablating

Catheter ablation of para-Hisian premature ventricular contractions (PVCs) still represents a challenge and is a compromise between success and inadvertent AV block. We describe a possible strategy to address PVCs from this location with high-amplitude His-bundle potentials at the site of earliest activation.
